LCVHF grant funds assisted the following numbers of uninsured or underinsured, low-income patients to obtain healthcare:
Prescription vouchers: 376
Dental Vouchers: 111
Transportation Assistance: 15
The Patient Assistance program speaks directly to CHAS’s mission and values, as we work diligently to address each patient’s needs. Our goal is to address the needs of the whole person. This is what sets CHAS apart. We understand that social determinants of health impact a patient’s physical and mental wellbeing. A patient recently told one of our SoCHAS PRCs that it was the PRC position that made CHAS unique. Most doctor’s offices do not work to help patients find resources to overcome barriers to achieving good healthcare in the same way that CHAS does.
